Registration

New Patients

To register at the surgery, please ask Reception for a registration pack. You will be asked to complete a form for NHS Contractor Services, this will enable us to gain access and transfer your medical records. You will also need to fill in a Health Questionnaire to help us provide you care before your medical records become available to us. Please be advised you will need a form of ID and proof of address when registering. If you haven't got either, please speak with a member of our reception team.

The following patients will be asked to attend a New Patient Check with our Nurse before registration is complete:
    •    Over 16 years old
    •    Born outside the UK

Registration for new/temporary patient between 10.30am – 11.30am and 5pm to 5:30pm MONDAY – FRIDAY. If this time is not conveinent please speak with a member of our reception team.

Online Services

You can register for online service to book APPOINTMENTS ONLINE , order REPEAT PRESCRIPTIONS ONLINE or access to MEDICAL RECORDS ONLNE.

Patients can view on-line, export or print summary information from their medical records relating to medications, allergies, adverse reactions and additional data that the doctor has agreed the patient may have access to.

Out of Area Patients

The practice registers out of area patients.

Out of Area patients are patients who live outside the practice area. The patient can see a Doctor or medical staff but the practice has no obligation to provide home visits or out of hours services when the patient is unable to attend their registered practice.
